I have found out that ANY burning blade enemy will drop the Lieutenant Insignia. We also found out that its in the Last room, aka big open room /w water in the middle, the one that is over looked by the treasure chest on the highest ledge...

Correct. ANY Burning Blade. I was out killing Fizzle at 41,25 for the quest "Dark Storms" and on my 2nd mob working my way up to him it dropped.
the insignia will drop from anyone in Skull Rock except the pets. It can drop on the first guy in the entrance to the caves (where I got my first insignia btw). The only reason I've found to continue into the caves are for difficult and dangerous xp, and to get the burning eye from the named one in the end of the caverns.
I noticed that you keep the insignia after you finish this quest. Do you need it for other quests or anything else?
I have been through this cave seven times and no drop yet, don't worry, it's not just you
I went through the cave 7+ times, killed everything in there , + no isignia, it was driving me crazy!!! Everyone in party kept saying it should drop + how they got it on the 2nd kill!!!! Was getting very frustrated!!!
Is this quest bugged or am I just jinxed????
I really, really want 2 abandon this one.......

The Time-Constrained Player can't (and won't) put up with low drop-rate quests usually but that wasn't my problem here. My 12 Shaman had no problem with the mobs (even phantom adds) until one random fight with a VW where a 14 named jumped me and killed me in one hit (at half health...). He wasnt an elite either. OK, but even that wasn't the problem.
The Tombstone Tavern was about three time zones away, so when I got back, everyone had rezzed and invited all their friends. It was like the Lexington Avenue Line at rush hour. Tried rezzing in corners and under water three times. Each time, I was jumped by two mages, three others and (I'm pretty sure) the entire Philadelphia Philharmonic Orchestra.
I don't mind farming through mobs for a quest (to a point). I don't even mind quick respawns (although no one has matched Ursal's record yet - he respawns before you're finished killing him) but there has to be a way to get back into the fight. Couldn't find the trick here.
Many thanks for the other posts, though; I'll try to find the Insignia elsewhere. Otherwise I will handle this quest with great abandon.
I've done this with a quite few characters, and they always get it on like their first-third kill.
Tonight, one character killed about 30 of them, I got sick of it and went to abandon the quest, and then picked it up again.
I went back and...it was the third kill that dropped one.
I think if you stay close to the entrance and/or first fork and look specifically for ones that are level 11, it'll speed up your hunt quick.
Go to the first fork in the cave and look in all three directions. Kill any level 11's you see from that spot. If you haven't found it by then, abandon the quest and retry. It can drop from others, but usually the first 11 you kill drops it.
This should prevent people from spending hours on this quest. ^_^
Was reading these horror stories on the drops (looking for other quest in cave) and was thinking about abandoning the quest without even bothering to try. Luckily i decided to do it anyway and had told myself i would clear cave 2 times. Got it on second kill. Lucky me!

the entrence to the cave is at 55, 9 in Durator. Hope that helped.
for those who dont get it, just keep killing, be persistant and you will get it, took me about 40 burning blades b4 i got it froma n apprentice

I didn't even need to go into the cave the 2nd guy at the entrance had the insigna....just goes to show u.
I've played this quest 3 times now and I always wonder why some people find the Lieutenant's Insignia on the first 5 kills of Burning Blades, others try it for hours. To my opinion this quest has a bug, I had always this problem in a party, when it drops for one member, there is no more drop for the other party members. When I logged out and in again I got it 3 times within 5 minutes. Strange but true.

it does suck if you dont get around to doing this quest until lvl 16 and nothing in the place gives you xp. I had to kill at least 100 of them before I got the drop.

And not once have I gotten the damned insignia. I've tried abandoning it and retrying it, and that didn't work. I need all the help I can get, so just throw your advice at me. Please, I really need it.

cleared the cave 1,5 times approx with a lvl 70 to help me.. means that we killed about 60-70 mobs .
It dropped from a normal mob, and i killed a guy called gazuu or something and he dropped a "this items begins a quest" item ^^
took me well over 100 kills to get this to finally drop. dropped off of a lvl 9 fanatic. almost leveled twice just from killing in skull rock.

[ul]I got it on my 60th kill, and yes once you get the burning eye quest you get the quest item in like the 4th kill. I got mine off some girl orc. I am a lvl 31 Blood elf rouge farming for rep, and I also got some good $$ from the line cloth that dropped! so once you get that started quest TURN IT IN RIGHT AWAY!
I think i killed about 400 mobs before i got it...i used to have a troll rogue b4 i got hacked and it dropped on the first kill. i dont think the drop from gazz'uz matters. u just have to keep killing them and itt will eventually drop...i did get impatient though and dropped it several times before i got it.

Just go into the Cavern... i had the item from a drop in the back of the cavern... just look, and you'll find it!
wow, that actually wasnt that bad! I was starting to lose hope, thinking ah HECK this is NOT a random drop at all is it, I must be in the wrong cave!
And then, after 20 mins or so, it dropped off a lvl 11 void walker guys master. KEEP UP HOPE!!!

me and my brother took teh q and whent to skull rock to get insignia. ususally i get it on like my 6th drop but this time we killed and killed and it never droped, i got about 14 bars of xp and then we gave up :/

I had to take down half the mobs in the cave, died about 3 times, all the mobs respawned, took down about the 3rd one inside the cave again, and he dropped it!
It was a pain in the ass, but still worth the loot.

this drop took me four hours. then i logged out then in again and it dropped straight away. hope this helps. i think the quest is bugged.
when i come in the cave i alwise aggro like 3 or more of them... i mean WTF i cant pass this q

Cleaned the area of mobs myself on a lvl 40 and took forever for the insignia to drop.
Took about 1.5 hrs of killing everything in the cave, but the insignia did finally drop for me. Must be completely random as to how often it drops, since some say first kill, some say hours, and some say days. It does drop tho, I got mine.
Logging out seemed to work nailed it on the first kill, kudos for the advice
