Following the instructions listed here, I killed Fallout twice and got the dagger on the second try.
Simply stealthed in, got to the edge overlooking the floor below, unstealthed, and jumped onto the silver gear. Waited for combat flag to drop (due to being spotted by mob on overlook when I unstealthed), sat and ate for a minute, then jumped off the gear.
Killed the closest mob to the gear (ooze or slime or something), restealthed and waited for Fallout to come by. His path takes about two minutes. With my back to the outer wall, looking at the silver gear I landed on, he came through and I jumped him. No adds and he was an easy kill.
First time, got the boots (they are BoP). Vendored them, but probably should have offered to let a friend or guildie come get them instead. I then invited a random guildie to join my group, promoted him to leader, left the group, then logged. When I logged back in, I was standing at the instance portal.
Second time, after killing the first ooze I forgot to stealth, so I was spotted by Fallout when he came by and he got first shot. It was still an easy fight - no evasion, no potions. The dagger dropped and is a nice complement to my Claw of the Shadowmancer (both 1.90 speed 27-51 dam). I'd like a slower, heavier mainhand but for now it's a nice upgrade.
The longest part of this whole shebang was flying to IF and running to Gnomer. Both kills and getting the dagger took less than 15 minutes.
