It is very possible for the Horde to catch this fish (as well as the Alliance to catch it's Horde counterpart - Old Crafty http://thottbot.com/i34486)
The only catch (no pun intended) is getting into Ironforge (or Orgrimar) Best time is off hours late at night or early morning...find a corner (I've posted a screenie of best location I've found)...and hope no one sees you :P

but it sucks because they need to make fishing poles 1h so you can hold your prized fish in the offhand

anything under 200 casts is nice IMO... it took me a total of 12 hours nonstop fishing (over the course of a few days), I went back there and got him on my 3ed try!
Took about 200 casts to get him the first time, then I accidentally pushed new cast and he was gone :S now after 870 fishies no luck ;(

When power-fishing, to avoid this problem, set your INTERFACE > CONTROLS > AUTO-LOOT on!!!

I had 20 minutes to kill before going to work. So, I thought I'd do a little to get my fishing up a bit (240 then) and caught Old Ironjaw after about 15 casts. I've been looking for him for about six months since my noob brother caught him and gloted on me.
The only downside was that I was so excited that I had to change my pants before going to work....
I must have the worst luck in the world. I've been fishing The Forlorn Cavern for days, over 2700 casts between getting killed and running back (I'm a Tauren). I've caught everything on the list at least twice. I keep telling myself just a few more casts, then I can go do something productive......
I've discovered you can actually fish while sleeping, so I lay down against the back wall and fish away, until I'm noticed. Is it possible this guy can only be caught in certain area's of the pool?

I actually caught this in the pond outside of where mages port you in. I had gone there specifically to catch Old Ironjaw, and caught it within 25 or so casts.
Now for the hard part... Getting into orgimmar to catch that rare fish... *sigh
There are no fish in the mage pool any longer.
Go in a tux set and be really, really patient.

70 BE pally in IF fishing and this dick of a druid kept killing her despite our best efforts to persuade him to stop it. Now the horde will kill us in Orgrimmar for it :(
Just some stats for comparison. I'm Alliance (Night Elf), with a fishing skill of 395 (with my fishing pole). After several hours of fishing the forlorn caverns I decided to switch to the mystic ward.
Not sure if I was lucky or if the change in location was the cause, but I would recommend going to the Mystic Ward first.

How could you fish in the Mystic Ward? I tried and it says "there are no fish in here".

I got this, as a Horde, by hiding behing a pillar and fishing in the lava. Took 2-3 hours and gave me the Old Ironjaw achievement. Now for the easy bit, fishing for the equivalent in Orgrimmar : )
This is 100% the way to go for horde. TY SO MUCH FOR MAKING IRONJAW EASY!!! Bring some camo elixers to hide from hunters, and get into lava. I perfered to get behind the lava wheel, sandwich between wall by jumping into saftey from lava, then they cant see your big red name either. little tougher to fish, but no1 can see u. FORLORN CAVERN IS DEATH, AND U WILL BE MERCILESSLY CAMPED/SLAUGHTERED BY ALLY
It took me around 700 casts to get this on a PvE server (I'm a Bloodelf priest).
Kept corpse running untill i made it to Forlorn Caverns and got to the far back of the cave.
I found that most allies respond well to friendly/non threatening emotes, especially fellow alliance fishers that can occasionally convince others to leave you alone:

/beg
Fishing in Forlorn Caverns will most likely get you killed from time to time, so this is done best late at night or early morning.
If they start focusing too much on you just log out and try later.

I'm human, and whenever I see hordies fishing in IF, at the back of the Forlorn Cavern, I try to /smile and /greet them (I /hug too if it's a female tauren - they're so cute!). I never once laid a finger on them.
I take pride in not taking advantage of their lack of armor and weapon (they have a fishing pole equipped). Yes, this is a computer game, and we can behave like grown ups in it.
Just let them fish.

I'm the same, I feel sorry for the Hordies since it's a right pain to get Old Ironjaw, compared to how easy it is for Allys to get the Orgrimmar fish. I always leave them in peace to fish in IF. Shame other's aren't so considerate..

Its not auto loot the screws it up (I have auto loot); its not pressing "ok" to bind it to you that screws you up. I had the same freaking issue yesterday. After 1130 fishes, I finally got him; but I did not press "ok" to bind item to me. Instead, I was so used to casting that I pressed it without thinking. DOH! Now I have 2234 fishes and I still have not seen him since the first catch. This is crazyyyyyy!!!!!
All my casts have been in IF. The weirdest catch I ever got from there was an item called "Bloated Toad". It was a toad that acted like a clam. You need to click on toad to get the gem it ate. I looked everywhere online to see if there was info on it, but I couldnt find any. Since IF are low level fish, the gem I got was a tigerseye. After more than 2000 casts, I never saw that item again. I just started leveling my fishing skill. I went from 0 to 375 in 2 days (Yesterday and today).
It was a "bloated mudsnapper".

Crocolisk exploit DOES NOT WORK on Old Ironjaw. I tried it myself this morning.
Yes, the crocolisk will drop every cast, even in Ironforge, but Old Ironjaw's drop rate does not shoot up like Old Crafty's does.
Anyways, 2000 casts deep and I'm pretty sick of this...
Even Mr. Pinchy dropped easier than this guy does. :(
I'm in the back of the Forlorn Cavern pool. I'm not getting hassled by the Alliance, thank god.
I've been in IF for two days now, trying to fish this guy up. I've spent equal amounts of time in the Forlorn Cavern, the Mystic Ward pool, and the lava. I'm guessing the drop rate for Old Ironjaw is the same regardless of where you fish as long as you cast off somewhere in Ironforge. I'm gonna spend the rest of my time fishing in the Mystic Ward.
So far I'm at: 1977 total catches
EDIT: now I'm at: 2500 total catches
...and no Ironjaw.
EDIT: I finally caught him after 2541 fish!!!!!
Caught him today (Tuesday) right after the server restart after only a dozen casts. Caught in the Mystic Ward pool.
Been trying to get him as Horde in IF too.
The trick to do this unseen is to jump into the lava at the Great Forge, die, and run back.
When you get the message saying you can res, walk backwards between the wall and the lavawheel.
When you can't go any further, jump backwards and you will land just above the lava.
Res and use camo potion to hide from the minimap.
You can now fish between the wall and the lavawheel.
It's a great spot where i could fish undetected for 4 days.
Note: When you get the achievement saying you have caught xxx fish, nearby Alliance will see that too and they will kill you....

Been trying to get this fish while leveling my last 150 skill points. So far, over 1500 casts and nothing. If you're going for the achievement, be prepared to spend countless hours of fishing unless you're in a really lucky day. Don't get fooled by all the positive comments you can read above here.
Fact is that most people going for this fish either gave up or went crazy doing so.

I'm Horde. I went to Forlorn Cavern in Ironforge and spent 4 hours fishing alongside allies - it was hillarious but great. After 436 fish, I finally caught ironjaw. The allies cheered for me when I hit 1000 fish and when I caught ironjaw.
Sure .. they killed me .. but I kept running back and respawning then casting .. everytime a new person showed up they'd kill me for a few times then get bored of it and decide it was much more fun to fish.
It was actually a pretty great night :)
My wife had been fishing in IF for about an hour, when I showed up to level my fishing.
Didn't really expect to get Old Ironjaw, but after just 10 casts, there he was.
My wife hates me now....
(But I consider it revenge from when she stole Lil' Timmy's White Kitten from me :D)
Add another retard who just got in the habbit of casting and auto-looting, that he didn't accept the bind when Ironjaw finally came up after hours and hours of fishing. I'm so mad at myself right now :(
Here I was, starting to fish the good old Ironjaw for the very first time, I had heard the stories of the old, broken fishermen who had been hunting the legendary uncatchable fish for years and years.
I had stocked food for weeks, months you could say, I was ready, I was determined, I was gonna catch the old Ironjaw, even if it would take the rest of my life, it was my dream, my destiny.
I sailed onto the forlorn cavern, made a sweet sweet campfire, pitched my tent up and started to fish with my loyal fishrod, Raw Brilliant Smallfish, Raw Bristle Whisker Catfish, 2 more Raw Brilliant Smallfishes, and...
On the fifth cast, less than 4mins on the fishing, I feel the strength of something huge coming from the water, I start the battle, I knew I had something big on my lure, not the Old Ironjaw I thought, its been 4 mins since I started, and I was prepared for weeks of fishing.
After the fight I knew the fish I had on my lure was losing his strength, and when I finally got the monster out from the fish and on the ground, I was stunned.
Enuff said.

I have 2 brothers and 2 cousins all with 70's and 80's gloating over how slow i am at leveling.
I was the first to get the deviate fish recipe without buying it now im going to be the first to get Old Ironjaw and Old Crafty!

I caught Old Ironjaw last week in the Forlorn Cavern with my gnome mage, level 18. I don't remember what time I caught it but I didn't have the problem many have posted about...There was no window asking to bind Old Ironjaw to me. I'm on Khadgar realm, fishing at (46, 15) in the Forlorn Cavern as I said. Took 120 casts but I got him.
Cannot Fish in the lava in Ironforge anymore, tried both sides using the wheel glitch. GAH! dunno if this is a 3.1 fix but either way cannot be done.
Aperently the Forlorn Cavern is now the only place you can fish in IF. Must have been after all the recent patches.
blizz should take away the "off-hand" part for fish so insanely bored folk like myself can get the pirate costume goin while armed with crafty in my left, ironjaw in the right or who knows, throw a massive tuna in one hand just to mix things up a bit...happy huntin

Here's the skinny:
You can NOT fish in the lava any more.
You can NOT fish in the Mystic Ward.
You can ONLY fish up Old Ironjaw from the Forlorn Cavern.
Be ready to fish for a while. It does NOT matter where in the pool you fish. What does matter is how much time you are willing to put into catching this thing.
I did like some others said and did a /lay toward the back wall. You CAN fish while laying down. They can still see a name above the water, but without a body to attract attn. to myself the allies did seem to attack less.
I got him after 504 casts over 2 different nights. Which is a lot less than other people have had to go through. You may be the lucky 1 cast or you may end up with Blizzard hating you and casting 2500 times.

1000 casts over 2 days and guildie showed up and caught it on 6th cast. Looks like I'll be fishing there a while longer...not so safe...I'm on my horde toon lol.
I'm Horde, and one thing I found helpful was to create an Alliance gnome or dwarf, run to Ironforge, and park the character at the Forlorn Cavern. If anyone gives me trouble I log out and attempt to talk it out with them via the new Alliance character.
