Since Ner'zhul is the joint Lich King (along with Arthas), it looks a bit like the Lich King's helmet/crown so I assume if he drops it in WOTLK it'll look like that :)

wrong i know all the history of the orcs this helment was ner'zhul's till he became the lich king (my guess is that he threw all his armor from when he was a shaman to make sure none find out he IS the lich king)
