I have just trained my lvl 38 hunter artisan first aid by completing the alliance Triage Quest in Theramore Isle. You get this from Ironforge (1st aid trainer) once you reach 225 skill in 1st aid.

Where is the 1st aid trainer in Iron forge?
I went to one but he didn't giveme anything.
I'm max'd at 225

Done it!
You need to get the Trauma quest from her and then complete the Triage quest from the doctor in the citadel in Theramore.

  There is no trainer for expert first aid. You simply have to buy this book:

  I always just bought mine from the AH for like a gold or so, though it is available to buy from a vendor.
