also just did it on my lvl 68 lock of scryers has 100/5800 hp left :P

You can get the achievement for falling into lava because it doesn't count as water. There's a place in BRD where you can fall that far into lava. I got the achievement by accident.
No, you do not have to make it out of the lava alive. I died instantly, but because I died from the lava, not the fall, I got the achievement.

ok. well i had a pally put [Hand of Protection] when i was flying at the height of the aldor elevator. it only lasts for a few seconds so as soon as it was on i unmounted and fell. i didnt loose any health and i got it. if u dont have a flying mount just get on the aldor elevator and when it is at the top jump off when u have the spell on you. it was really easy. =)

Just got it tonight, after a billion tries without success off the Scryers tier... don't jump forwards. When you are facing the centre of shatt, turn to the right (towards the Scryers bank) and aim for the grey part down there. It is just a fraction more of a jump... and achievement unlocked.

Easy for a mage ... jump off scyer/aldor tower and ice block as you fall.

Go to Stormwind harbor and jump off in front of the fountain

I am a 62 Mage. Went to the Scryer Elevator and tried that but kept getting stuck on the sides of the buildings for some reason, so I decided to go a little farther over on the Scryers tower and jump towards the "Lower City Market" area. Where I jumped is over on the platform by the Arena Vendor (just take a left when you get off the elevator...the spot I stood on was 59,71). I ran and jumped towards the market place and the hit my Ice Block and Ta-da!! I got the achievement. Kinda make sure you jump towards an open spot though! Good Luck!!

I needed an easy reproducable way of doing this with several chars, most of them with no flying mounts. What I got work consistantly is:

Go to thunderbluff, take the elevator closest to the graveyard up. When the elevator turns and starts descending, simply step off once your hand has reached the hight of the wooden ledge that leads to the elevator.

Meaning: place yourself on the elevator. Position the camera so that you can see your entire body and the wooden ledge to the right of you. When the elevator is going down, the ledge will go "up" on your body sort of like a thermometer ;)
Once it hits the height of the hand (on a tauren that is) take one step forward and fall. Hit the ground and Hooah!

Managed to reproduce this first time with 4 chars.

Another great thing with this is that if you fail and fall to far, the gy is just seconds away.

Remember to have your frostweave bandages with you and use one on yourself after the jump to get your Ultimate Triage achievement at the same time.

If you go in The Sewers of undercity, go to the dead end then jump. I did this as a level two and I diden't die, so this works great for those horde looking to get this achievment.

One possible place to get this is in dun niffelem. Just walk from the cliff from SoH flightmaster towards the valley of ancient winters.

If ya want the ultimate triage after this one then you must jump off of the cliff and immediately use the bandage after landing...

Jumping of a cliff in north-east Azhara works too. Coords are 75,10. You'll know you've found the place when you see a slime wandering near the edge. You may encounter several lvl 54 elites until you get there.

I did this as a lvl 50 warlock and it worked every time, no deaths (I jumped again with a friend, we both landed alive, and he did the ach too).

I just can't seem to get this one and I've tried many times.  Neither of the elevators in Shatt work.  I followed the suggestions above and nothing.  Nothing at Scryer's tier.  I tried the Aldor elevator suggestion and went from 13,400 HP to 1300 and didn't get it.  Seemed like it should have been high enough.  Is this bugged after the patch?  Does it do it after a certain percentage of health lost and not actually distance?  Different things seem to work for different people.  I've fallen from higher than what people here have said worked and survived but didn't get the achievement.  Do I need to lose more HP than that?  It's frustating me!  lol

I was playing as a new DK and doing one of the last quests where you get the frostwyrm (the DK was unholy and had the talent where you come back as a ghoul when you die) so anyway, the frostwyrm got killed and I fell from really high up, I died, but came back to life as a ghoul and got the achievment, so if your an unholy dk then this achievment shouldn't be too hard to get

You can get this in thunder bluff right before the left pathway to the elevator(i got this by falling on accident)

Go to Sunstrider Isle out past Silvermoon follow road to the Falthrien Academy follow up the LEFT side to the SECOND arching bridge jump over the side of that bridge..it works every time..I created several characters just to make sure..no matter what lvl you are if you are full health and jumped there you will get the Achievement.

Sunsail Anchorage in Eversong woods 32,68 approximately.
Jump from top lvl which is 3rd floor.  Stand beside the light on right (it's on the rim)side facing north west.

I got mine by accident when I fell off the cliff near Honor Hold. Got killed by a mob less than a second after I landed.

Also, jumping off the bridge into the lower city in Shatt works too. Got the Brewfest one that way.

I got this achievement accidentally while running to catch the boat in Stormwind. Mounted on horse, jumped near fountain, missed the ramp and landed on the catapult and survived. Missed the boat because I was tangled up in the catapult. Have tried to duplicate the feat with several other characters but no success.

A friend and I jumped our rogues off the Twin Colossus in Feralas and got the achievement but again could not do it with other characters. Note that rogues have extra fall resistance.

During Brewfest, while doing the Pink Elicks quest, my troll shaman accidentally fell off the main elevator platform at Thunder Bluff and got both the Going Down and Drunken Stupor achievements. Was able to duplicate this with several other characters including a human priest (Alliance).

Directions - On the first level of Thunder Bluff head toward the elevator and walk out on the right had platform. Stop on the last plank and turn to the right. Aim so that you will land at the base of the cliff, if you land on the grass you will die. Walk off the platform, don't jump.

Alliance can do this because there are no guards at the bottom of the elevator. When you step off the elevator you must immediately turn and walk off the platform. If you stand there working up your courage the guards will get you. They are fast - trust me.

Having had some success with my Horde characters I got interested in finding a better ploy for Alliance, so I went to Thottbot and found the post about jumping off the falls near Beggar's Haunt in Duskwood. Coords are 88,24. Climb onto the right hand rock at the top of the falls, stand on the highest point, turn toward the falls, aim just to the right of the little point at the edge of the rock, run and jump.

Some points to remember. Make sure you are fully repaired and at full health before you jump. Having damaged gear seems to make a difference. Get the Synthebrew Goggles at the Brewfest and wear them. This will get you the Drunken Stupor achievement if Brewfest is still on but it also seems to keep your gear from being damaged by the fall.

if you're a low lvl alli, then jumping off the platform in exodar works perfectly. i used it to get not just the going down achievement but drunken stupor achievement from brewfest too =)

i got this by glitching throught the undercity elevator pritty easy all u got to do is as the elevator is goin up keep on jumping at the wall and hope u all trough at the right time :)
