I have found out that ANY burning blade enemy will drop the Lieutenant Insignia. We also found out that its in the Last room, aka big open room /w water in the middle, the one that is over looked by the treasure chest on the highest ledge...

Correct. ANY Burning Blade. I was out killing Fizzle at 41,25 for the quest "Dark Storms" and on my 2nd mob working my way up to him it dropped.

Lol I guess I got pretty lucky, I got it when I was doing Dark Storms (from the first burning blade I killed there).
