Location is 60, 72 for the Moonwell.  It shows up as a blue dot on map.

Easiest way to get there is to follow the river south from the bridge over the Falfarren River.  Go down the river a short way, then bear right.

Once you are standing in the Moonwell, right-click the phial.  It will fill up and you are done.
