    I've played this quest 3 times now and I always wonder why some people find the Lieutenant's Insignia on the first 5 kills of Burning Blades, others try it for hours. To my opinion this quest has a bug, I had always this problem in a party, when it drops for one member, there is no more drop for the other party members. When I logged out and in again I got it 3 times within 5 minutes. Strange but true.

      Played this with a group of 5 last night. If it was bugged, it doesn't appear to be anymore. We spent about 30-45 minutes (not exactly sure)in there to get 3 insignias (two of the members had already finished the quest and turned in to the next step.) We were using standard group loot.
